Obstetrics and Gynecology is a branch of medicine that specializes in the care of women during pregnancy and childbirth and in the diagnosis and treatment of diseases of the female reproductive organs. Today’s topic is about Cancer Cervix. Let’s learn about it a bit more in detail, shall we?

Cancer Cervix

  • Average age of diagnosis: 49 years

Most common cause of death in cervical cancer is uremia

Risk Factor:

  • HPV Infections
  • Multiparity
  • Early age of sex intercourse
  • Multiple sex partners
  • Vaginal birth
  • STD ( HIV/ Herpes)
  • Immunocompromised
  • Smoking
  • Combined oral contraceptives

Histopathological types

Squamous carcinoma – 80 %

Adenocarcinoma        – 20%

Serous Carcinoma

Clear cell carcinoma

Mucinous Carcinoma

Staging

Clinical + Imaging

                Sigmoidoscope

Stage I: same place – I A: less than 5 cm

                                    I B: more than 5cm

Stage II: Adjuvant areas

Stage III: Uterus ( parametrium) , ovary , endometrium – peritoneal

Stage IV: Distant

 

Treatment   – Surgery

Staging with their treatments

IA – A1:  < 3 mm – microinvasive – Conization enough 

        A2:  3 – 5 cm – Radical hysterectomy

IB – B1 – less than 2cm – Trachelectomy or Radical hysterectomy           

        B2 – (2 – 4 cm) – Radical hysterectomy

        B3 – 4 cm – Chemoradiation

II A –    upper 2/3 vagina    A1 – < 4 cm

C2 – Paraaortic nodes

IV a – Bladder / bowel mucosa 

IV b – Distant metastasis

 

In short, I B3 , II a2, II b  , III, IV – Inoperable Chemoradiation
